If you want Zenput to create a form for you please follow the steps below.
To begin, navigate to the Forms section of your Zenput account.
Click 'New Form' in the top right corner of your home screen.
A window will appear asking if you would like to create a new form from scratch or upload a form to Zenput support to create on your behalf. Select 'Blank Form'.
You can upload pdf, xlsx, and/or .doc. Under Notes and Comments please be as detailed as possible about what you would like in the form and/or how you would like the form formatted with sections, instructions, what kind of questions, any scoring, etc.
If you select 'Upload Form' you can upload a form for creation on your behalf by Zenput, please keep in mind that this option will usually take 5 business days from the time it is uploaded until it is available in your account. This is based on the quantity of forms and the amount of questions it includes.
Once the form has been uploaded you will receive an email from us that the form has been copied to your account and is available to distribute or assign, from the Web-App under Forms.
